CLASS REGISTRATION FORM
CLASS REFUND/CANCELATION POLICY
If class is canceled for any reason by Carlee Justis, a full refund will be given. If the minimum number of students is not met then class will be canceled. Withdrawals made after the class has started will not receive a refund. Returned checks will be levied $25.
